Includes respite, advanced respite, homemaker/chore, personal care, advanced personal care, meals, transportation, adult day care, and other services to help with activities of daily living.

Eligibility

Elderly or disabled, Medicaid eligible and assessed by the Division of Senior & Disability Services to qualify for nursing home placement if services paid for by government funds.  None if private pay.

Regulated by the Division of Senior & Disability Services and the MoHealthNet Division.

Home Health Care

Eligibility

Elderly or disabled, homebound, have a skilled health care need that requires intermittent care.

Adult Day Care

Eligibility

Elderly or disabled, homebound, have a skilled health care need that requires intermittent care.

Hospice

Includes interdisciplinary team of health and social service professionals that provide necessary services including nursing, medications, personal care, respite, ministerial and social work.

Eligibility

Have a life expectancy of less than 6 months.

Authorized by the physician and patient.

Regulated by the Department of Health - Bureau of Home Care and Rehabilitation Standards.
